Job summary

Paradigm Corp. is seeking a compassionate LPN Post-Acute Care Coordinator to support injured workers through coordination of post-acute care services.

This fully remote, full-time role works Monday through Friday from 12:00 PM to 8:30 PM Eastern Time, with an hourly pay range of $28.00 to $30.00. You will collaborate with healthcare providers, case managers, and insurance professionals to facilitate care transitions and ensure positive outcomes in the recovery journey of injured workers.

Qualifications

• Current LPN state license in good standing.
• Minimum of one year of nursing experience.
• Knowledge of injured worker rights and PHI confidentiality.
• Excellent verbal and written communication, teamwork, and organization.

Responsibilities

• Process referrals for injured workers for Post-Acute Care.
• Provide staffing for Post-Acute Care referrals.
• Communicate progress with clients regarding injured workers.
• Address inquiries from Adjusters, Case Managers, Providers, and Injured Workers.
• Coordinate discharges and post-discharge services with facilities.
