About Us

ChartSpan is the largest chronic care management (CCM) service provider in the US. CCM programs focus on patients with multiple (two or more) chronic conditions expected to last at least 12 months. We help providers stay in touch with and meet their patients’ healthcare needs between office visits.

Role Overview

The LPN Personal Care Coordinator cares for patients in our program, working with their care team to address and manage chronic health issues through outbound calls. The role also involves assisting patients in obtaining resources to improve their health, happiness, and longevity. We seek passionate nurses who are advocates for patient well-being and aim to build ongoing, collaborative relationships to enhance lifestyles.

Shift Details

This is a day shift position, operating Monday through Friday. It is a full-time, remote role with benefits, and all necessary equipment is provided.

Responsibilities
  1. Conduct monthly care management conversations via outbound calls to address healthcare needs.
  2. Communicate available resources and services to patients.
  3. Identify patient-specific problems, set goals, and implement interventions.
  4. Maintain compliance and proper documentation of medical histories, medications, immunizations, allergies, surgical and family histories.
  5. Be aware of patient circumstances that may require adjustments to care plans and address barriers to care.
  6. Provide health education as appropriate.
  7. Escalate patient concerns to the triage nurse team.
Performance Metrics
  1. Proper utilization of resources.
  2. Scheduled call percentage.
  3. Quality scores.
  4. Adherence to scheduled hours and attendance.
Qualifications
  1. Active, compact LPN/LVN license.
  2. Adherence to clinical protocols and procedures.
  3. Flexibility and ability to switch tasks effectively.
  4. Team-oriented attitude and willingness to assist colleagues.
Work Environment
  1. Fully remote with provided equipment.
  2. Professional office environment.
  3. Use of standard office equipment such as computers and phones.
Required Education
  • LPN/LVN degree from an accredited nursing program.
