Your Role

• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients' progress and work with the physicians to ensure quality patient care.

Qualifications

• Active LPN license (compact)
• Some office experience required — geriatrics or primary care/internal med highly preferred
• Ideal candidate will be bilingual (English and Spanish)
• Ability to sit and talk on the phone for long periods
• System requirements: Windows i5 (or newer) processor, Windows Pro (not Home). Laptop or desktop acceptable.
• Must live and work full time in the US — remote work within the US only.

The Pay

• Starting at $22.85 per hour (40 hrs/week)
• Hourly rate plus monthly bonuses. High performers may earn up to $1000 bonus per quarter.
