Slate Health is expanding our Chronic Care Management (CCM) team and is looking for compassionate and organized Licensed Practical Nurses (LPNs) who enjoy helping patients with chronic medical conditions.

This hybrid role lets you work primarily from home with occasional in-office collaboration or training in Florida, with options for part-time or full-time schedules to fit your availability. You will support patients between visits and help keep care plans on track.

Qualificações

• Active LPN license in good standing.
• Comfortable using basic computer systems and documentation.
• Strong communication and organization skills.
• Reliable home office setup.
• Ability to attend occasional in-person meetings or training in Florida.

Responsabilidades

• Speak with patients and caregivers by phone.
• Help patients stay on track with medications and care plans.
• Coordinate with physicians and nurse practitioners.
• Document patient interactions.
• Support patients managing chronic conditions.
