Job summary

Bradford Health Services is seeking a Licensed Practical Nurse to provide direct nursing care within treatment and recovery settings. The role involves administering medications, monitoring patients, and collaborating with multidisciplinary teams to support individualized care plans.

Ideal candidates will have an LPN license and experience in behavioral health, with strong patient education skills and knowledge of EHR systems.

Qualifications

Current LPN license in the state of practice.
Strong nursing principles and patient education techniques.
Ability to comply with healthcare regulations and protect patient confidentiality.
Preferred: experience in behavioral health or substance abuse settings.
Preferred: bilingual abilities and EHR familiarity.

Responsibilities

Provide direct nursing care to patients, including administering medications and monitoring vital signs.
Support patients through recovery by applying knowledge of substance abuse and behavioral health principles.
Educate patients and their families about treatment plans, medication management, and healthy lifestyle choices.
Collaborate with healthcare professionals to develop and implement individualized care plans.
Maintain accurate and detailed patient records in compliance with healthcare regulations and standards.
Observe and report changes in patient conditions to registered nurses and physicians promptly.
Assist in creating a safe and supportive environment that promotes patient well-being and recovery.
