Qualifications

• High school diploma or GED required.
• Current, unrestricted Massachusetts LPN license required.
• At least one year of nursing experience preferred.
• Ability to apply the nursing process and provide safe, effective care.
• Knowledge of medication safety, documentation, and HIPAA compliance.

Responsibilities

• Provide compassionate, person-centered LPN care under nursing leadership.
• Collect and communicate health status, symptoms, and response to treatment.
• Implement nursing orders and maintain plans of care.
• Complete health assessments, progress notes, and medication records on time.
• Safeguard medications and ensure records comply with laws and policy.
• Educate individuals and families about health, treatment, and prognosis.
• Maintain HIPAA confidentiality and accurate shift reporting.
• Coordinate with medical staff and external facilities to ensure continuity of care.
• Participate in meetings, training, and quality improvement activities.
