Job summary

Alder is seeking a Life Care Manager to join our team in the Cambridge/Boston area. This remote full-time role focuses on coordinating care, assessing needs, and supporting families through aging journeys.

You will conduct psychosocial assessments, build relationships with clients and caregivers, arrange medical appointments and transportation, and advocate for client-centered care. A licensed professional with 3–5 years’ experience and ALCA credentials is preferred.

Qualifications

• Advanced degree in social work, nursing, geriatrics or related field and active licensure.
• 3–5 years' experience with older adults and families.
• Strong clinical and communication skills.
• Ability to drive and have reliable transportation.
• ALCA membership and licensure maintenance.

Responsibilities

• Conduct psychosocial assessments and document findings.
• Develop client-centered care recommendations for home and facility care.
• Coordinate medical appointments and transportation as needed.
• Provide ongoing advocacy and family education.
• Attend supervision and ongoing professional development.
