Qualifications

• Advanced degree preferred and licensure required in social work, nursing, geriatrics, mental health, or a related field.
• 3–5 years’ experience working with older adults and families.
• Excellent clinical and communication skills; strong organizational ability.

Responsibilities

• Conduct professional assessments of older adult clients and families.
• Develop client-centered care recommendations and communicate them clearly.
• Coordinate and oversee home care, assisted living, or nursing home arrangements.
• Provide advocacy and ongoing support to clients.
• Arrange medical appointments and transportation as needed.
• Document billing and interventions daily.
• Attend supervision and professional development activities.
