Job summary

Alder, a trusted guide for families in Greater Boston, seeks a Life Care Manager to join our team remotely. You will assess older adult clients, coordinate care, and advocate for families, drawing on your clinical skills and ALCA-aligned practices.

You will build relationships with clients and providers, arrange appointments and transportation, support families through transitions, and maintain licensure while contributing to a compassionate, multidisciplinary approach.

Qualifications

• Advanced degree in social work, nursing, geriatrics or a related field with licensure.
• 3–5 years’ experience working with older adults and families.
• Excellent clinical and interpersonal skills; strong communication capabilities.
• Highly organized; able to manage fee-for-service caseload.
• Maintain ALCA membership and state licensure as required.

Responsibilities

• Conduct professional assessments of older adult clients and families.
• Develop client-centered, evidence-based care recommendations.
• Coordinate and oversee care options (home care, facilities, transportation).
• Provide ongoing advocacy and problem-solving support for clients and families.
• Document interventions and participate in supervision and meetings.
