Qualifications

• Advanced degree and licensure in social work, nursing, geriatrics, mental health, or related field are preferred/required.
• 3–5 years’ experience with older adults and families.
• Excellent clinical skills; strong knowledge of family dynamics.
• Strong organizational skills and ability to manage fee-for-service caseload.
• Outstanding interpersonal and communication skills.
• Proficient with electronics (smartphone, computer).
• Ability to drive with vehicle access.
• ALCA membership and certification as required.
• Massachusetts licensure maintenance.

Responsibilities

• Conduct professional assessments of older adult clients and families.
• Develop relationships with clients, families, and care providers.
• Communicate client-centered care recommendations to clients and families.
• Assist in implementing and overseeing care recommendations.
• Provide client-centered problem-solving and advocacy.
• Arrange transportation and accompany clients to appointments as needed.
• Provide support to families coping with loss, dementia, or illness.
• Document billing and interventions daily.
• Attend supervision and staff meetings.
