Qualifications

• 3–5 years' experience working with older adults and families.
• Excellent clinical skills; knowledge of family dynamics and systems.
• Outstanding interpersonal and communication abilities.

Responsibilities

• Conducts comprehensive assessments of clients’ needs across medical, financial, cognitive and social domains.
• Assists clients and care teams with aging, disability, services, and long-term planning decisions.
• Creates goal-oriented care plans and revises them as goals change.
