Integral Health is seeking a Licensed Therapist/Behavioral Care Manager for a fully remote, full-time role in New York. You will provide direct behavioral health therapy via telehealth while coordinating care with primary care teams under the Collaborative Care Model.

This position emphasizes short-term, measurement-based interventions with evidence-based practices such as CBT and motivational interviewing, thorough documentation, and collaboration with PCPs and psychiatric consultants.

Qualifications

• Master’s degree in a relevant behavioral health field.
• Active, unrestricted New York LMSW, LCSW, LMHC, or LMFT license.
• At least two years of experience providing mental health assessments and direct therapeutic interventions.
• Ability to maintain a consistent full-time schedule, including two evening shifts per week.
• Experience delivering evidence-based interventions such as CBT, motivational interviewing, problem-solving therapy, or behavioral activation.

Responsabilités

• Provide direct, patient-facing behavioral health therapy through telehealth.
• Deliver brief, evidence-based interventions using CBT, motivational interviewing, problem-solving therapy, and behavioral activation.
• Screen and assess patients for mental health conditions, substance use, safety risks, and SDOH.
• Use PHQ-9, GAD-7, C-SSRS, and SDOH screenings to guide treatment and monitor progress.
• Develop patient-centered care plans with SMART goals and monitor progress.
