Job summary

Nourish is expanding its clinical model to include behavioral health care as part of our fully integrated metabolic care offering. We are seeking Licensed Therapists for both full-time and part-time roles, primarily delivering care remotely.

You will partner with dietitians, NPs, physicians, and care operations to deliver longitudinal support, addressing behavioral health and motivation to drive lifestyle changes. Our onboarding supports credentialing and scheduling to start rapidly.

Qualifications

• Active, independent clinical license in the U.S. (LCSW/LPC/LMFT or equivalent).
• Experience with telehealth delivery.
• Experience supporting behavioral health and chronic disease management.
• Ability to work in a multidisciplinary, team-based telehealth environment.
• Commitment to evidence-based practice and long-term behavioral change.

Responsibilities

• Provide direct behavioral health care using an evidence-based, patient-centered approach.
• Conduct virtual visits and manage a caseload per role expectations.
• Document encounters, coordinate follow-up care, and review care plans.
• Support care pathways by addressing behavioral, emotional, and psychological barriers.
• Collaborate with RDs, NPs, physicians, and care team members to integrate care.
• Use data and feedback to improve care delivery, outcomes, and coordination.
• Maintain high standards of integrity and patient confidentiality.
