Job summary

Ascend Healthcare, a fully remote psychiatric and behavioral health provider, seeks a Licensed Therapist or Counselor (LPC, LCSW, LMFT) to deliver telemedicine therapy to Community Health Center partners in Nevada. You must hold an active license in your state and be willing to obtain Nevada licensure.

Qualifications

• Active, unrestricted mental health license in state of primary residence and state of medical services being provided (Arizona).
• Comfortable working in a remote environment.
• Patient-first mindset.

Responsibilities

• Provides patient care modalities of evidence-based practices to best meet the clinical needs of the patients.
• These treatment modalities will be implemented in both the individual and group therapy setting.
• Review and signature of patient clinical psychosocial assessment, diagnosis, treatment plan, and consents as needed.
• Documents all services and patient activities appropriately to comply with best clinical practices and contracts.
• Conducts crisis intervention for the individual and family according to safety plans.
• Participate in regular case planning with the Multidisciplinary Team to ensure appropriate disposition.
• Engage in supervision and ongoing education to maintain competency and performance.
• Provides case management services to promote recovery and reintegration.
• Performs other duties as assigned.
