Ascend Healthcare is seeking a Licensed Therapist or Counselor (LPC, LCSW, LMFT) to provide remote counseling via telemedicine for patients of Community Health Center partners in Arizona. Clinicians must hold an active license in their state of residence and in AZ if providing medical services.

The role emphasizes evidence-based practices, crisis intervention, and collaboration with a multidisciplinary team to support recovery and stabilization for diverse patient populations.

Qualifications

• Active, unrestricted mental health license in state of primary residence and in AZ.
• Comfortable working in a remote environment.

Responsibilities

• Deliver evidence-based therapies (CBT, DBT, solution-focused, trauma, addiction therapy) in individual and group settings.
• Lead and engage patients in their treatment plans within telemedicine sessions.
• Review and sign psychosocial assessments, diagnoses, treatment plans, and consents.
• Maintain compliant clinical documentation and records.
