Job summary

Ascend Healthcare is seeking a Licensed Therapist or Counselor (LPC, LCSW, LMFT) to provide remote counseling via telemedicine for Arizona Community Health Center partners. Active AZ license and willingness to obtain relevant licenses in the primary residence state are required.

The role supports a remote-first practice with evening hours, comprehensive benefits, and ongoing professional development. In-person attendance may be required as needed.

Qualifications

• Active, unrestricted mental health license in state of primary residence and in state of medical services being provided (Arizona).
• Comfortable working in a remote environment.
• Willingness to work evening hours.

Responsibilities

• Provide counseling and therapy using evidence-based modalities.
• Deliver services remotely via telemedicine to patients of Community Health Center partners in Arizona.
• Maintain accurate documentation and case records in a timely manner.
• Participate in case planning with Multidisciplinary Team and supervision as required.
• Provide case management to promote recovery and community reintegration.
• Perform other duties as assigned.
