Job summary

Vynca is seeking a Clinical Lead Care Manager to join the Enhanced Care Management team in Riverside County, CA. The role blends in-person and remote care management, requiring travel up to five days weekly and residency within 20 miles of the territory.

The ideal candidate is an LVN with 2+ years in care management, strong communication, and the ability to coordinate with doctors, social services, and families to maximize client outcomes.

Qualifications

• Active, unrestricted California LVN license required.
• 2+ years of experience as a care manager, care navigator, or community health worker.
• Bilingual English/Spanish highly preferred.
• Valid driver’s license and reliable transportation.
• Proficient with Google Workspace and MS Office.

Responsibilities

• Assess member needs across physical health, mental health, SUD, social supports, and housing.
• Develop and manage client care plans and SMART goals.
• Provide services in office, telehealth, or field settings.
• Coordinate with providers and social services to ensure aligned care.
• Document care activities and complete monthly reporting.
• Travel within Riverside County up to five days a week.
