Job summary

FronteraCare in Colorado and New Mexico is seeking a CHW Lead to manage a remote, regionally dispersed team of Community Health Workers. You will mentor staff, oversee outreach, and ensure high-quality engagement and continuity of care for families receiving Diagnostic and ABA services.

You will align with NMDOH standards, monitor intake and waitlist metrics, coordinate referrals, and drive performance through data analysis and leadership collaboration across FronteraCare regions.

Qualifications

• Minimum 3 years as a CHW or care coordinator in health or social services.
• Active social work license per NM requirements.
• Experience mentoring or supervising staff.
• Strong remote-work communication and leadership abilities.
• Bilingual in English and Spanish.

Responsibilities

• Oversee day-to-day CHW operations and performance.
• Mentor CHWs to ensure engagement and care quality.
• Monitor onboarding, intake conversion, and waitlist metrics.
• Coordinate referrals, assessments, and family onboarding processes.
• Ensure documentation accuracy and regulatory compliance.
• Lead alignment with technology platforms for family engagement.
• Recruit, onboard, and train new CHWs.
