FronteraCare delivers autism diagnostic and ABA therapy services to children and families across New Mexico and Colorado. The role oversees CHW activities to ensure effective family engagement and care coordination while leading a remote team of CHWs through coaching and supervision.

The position requires a minimum of 3 years as a CHW, an active social work license (LSW/LMSW/LCSW NM), and bilingual English/Spanish.

Qualifications

• Minimum 3 years experience as a CHW or similar role.
• Active social work license (LSW/LMSW/LCSW per NM licensure).
• Bilingual in English and Spanish.

Responsibilities

• Oversee CHW activities to ensure effective family engagement and care coordination.
• Monitor performance metrics related to onboarding, service continuity, and waitlist management.
• Lead, mentor, and develop a remote team of CHWs through coaching and supervision.
