HealthCare Strategies (HCS) is seeking an experienced Licensed Clinical Social Worker (LCSW) to join our Musculoskeletal (MSK) Care Management Program in a fully remote role. The LCSW will deliver evidence-based behavioral health interventions for MSK patients whose recovery is affected by psychosocial factors, working entirely via telehealth.

Competenze

• Active, unrestricted LCSW license in at least one state.
• MSW from CSWE-accredited program.
• 3+ years of post-licensure clinical experience.
• 2+ years in chronic pain management or related behavioral health settings.
• Experience delivering CBT for chronic pain or ACT.
• Experience using electronic health records or care management platforms.
• Comfortable providing telephonic and telehealth-based services.

Mansioni

• Conduct comprehensive psychosocial assessments for referred MSK members.
• Administer and interpret validated assessment tools.
• Develop individualized behavioral health treatment plans.
• Deliver CBT for Chronic Pain (CBT-CP) interventions.
• Provide Pain Neuroscience Education (PNE) and behavioral coaching.
• Address depression and anxiety impacting MSK recovery.
• Support return-to-work readiness through behavioral activation and coaching.
• Coordinate care with primary care physicians and other providers as needed.
