Job summary

CareStar, Inc. in Dougherty County, GA, is seeking an ISC Clinical Supervisor to assess needs, develop care plans, and connect clients with essential services across the Dougherty County area and surrounding counties.

The role requires an RN/PA/PT/OT or equivalent, with at least 2 years in acute care or related rehab settings, and a minimum of 3 years in home and community-based case management. Remote with field visits; travel may be required.

Qualifications

• RN/PA/PT/OT or equivalent mid-level healthcare provider required.
• Minimum 2 years in acute care, ICF/ID long term care, or medical rehabilitation.
• Board Certified Behavior Analyst, Psychologist, or LICSW/ LCSW/ LPC preferred for Behavioral Supervisor.
• At least 3 years of home and community-based case management for individuals with disabilities.

Responsibilities

• Provides information and assistance to link participants with medical, social and/or education providers and services.
• Conducts health, safety, social and environmental assessments.
• Serves on work groups or committees and attends meetings as requested.
• Demonstrates effective organizational and communication skills.
• Experience in care management/coordination preferred.
