The role combines remote work with field visits to assess needs, develop care plans, and connect clients with essential services, ensuring quality care coordination. The position requires clinical supervision capabilities for medical or behavioral health tracks, with experience in home-based case management and knowledge of relevant programs.

Qualifications

• Experience in home and community-based case management is preferred.
• Strong organizational and communication skills are essential.
• Knowledge of Medicare/Medicaid, waivers and community resources is a plus.

Responsibilities

• Links participants to medical, social and educational providers and services.
• Performs health, safety, social and environmental assessments per guidelines.
• Participates in work groups, committees and task forces as needed.
• Demonstrates effective organizational and communication skills.
• Experience providing care management/coordination preferred.
