CareStar, Inc. is hiring an ISC Coordinator for Cobb County and surrounding counties. You will assess client needs, develop personalized care plans, and connect individuals with essential services, working remotely with periodic field visits.

The role emphasizes care coordination, collaboration with healthcare and community providers, and a commitment to improving lives through supportive case management services.

Qualifications

• Bachelor's degree in human services or related field required.
• Two years direct service experience for DD/IDD or related disabilities.
• Two years’ healthcare or related setting experience preferred.
• Experience completing assessments, service plans, and monitoring.
• Strong organizational and communication skills.

Responsibilities

• Link participants with medical, social, and educational providers or programs.
• Conduct health, safety, social, and environmental assessments per guidelines.
• Participate in work groups, committees, and meetings as requested.
• Travel as needed and maintain reliable transportation and valid driver’s license.
• Adhere to CareStar policies and maintain confidentiality.
