Job summary

CareStar, Inc. seeks an ISC Clinical Supervisor covering Bibb County, GA and surrounding areas. This remote role includes field visits as needed to assess needs, develop personalized care plans, and connect participants with medical, social, and education services.

You will collaborate with families, coordinate home and community-based care, and ensure alignment with CareStar policies and community resources. Strong organizational skills and confidentiality are essential.

Qualifications

• Registered nurse, physical or occupational therapist, physician assistant or other mid-level healthcare provider.
• Minimum of 2 years’ experience in acute care, ICF/ID long-term care, or medical rehabilitation.
• Must have 2+ years professional experience with individuals with complex behavioral issues in a behavioral health setting.
• Three years’ home and community-based case management experience for individuals with disabilities.
• Strong organizational and communication skills; knowledge of Medicare/Medicaid/waiver programs.
• Integrity, ability to work independently, confidentiality and adherence to policy.

Responsibilities

• Link participants with medical, social and education providers and services to meet goals.
• Conduct health, safety, social and environmental assessments per guidelines.
• Participate in work groups, committees and meetings as requested.
