Concerted Care Group is seeking a Remote Licensed Mental Health IOP Therapist to join our collaborative, interdisciplinary team. You will lead IOP groups, conduct individual and family sessions, and contribute to integrated treatment plans for adults and adolescents.

This role requires strong CBT/DBT/ACT skills, trauma-informed care, and MD licensure (LCPC/LGPC/LCSW/LMSW). EMR experience and a commitment to ethical, patient-centered care are essential.

Qualifications

• Minimum 2 years of direct clinical experience providing mental health treatment.
• Licensed as LCPC, LGPC, LCSW, or LMSW in Maryland.
• Master’s degree in counseling, Social Work, Marriage & Family Therapy, or related field.
• Strong working knowledge of CBT, DBT, ACT, Motivational Interviewing, and trauma-informed care.
• EMDR experience is preferred.
• Comfort using Electronic Medical Records (EMR).
• Commitment to ethical, patient-centered care and professional growth.
• The ideal candidate is comfortable working with both adolescents and adults.

Responsibilities

• Lead Mental Health IOP Groups and facilitate individual and family sessions.
• Develop and maintain person-centered, trauma-informed treatment plans.
• Conduct comprehensive mental health assessments and level-of-care determinations.
• Educate patients on diagnoses, treatment approaches, and symptom management.
• Design and deliver psychoeducational and skills groups on emotional regulation and coping."
• Collaborate with psychiatrists, nurses, PRP, and other therapists to provide integrated care.
• Participate in treatment team meetings, case consultations, and discharge planning.
• Coordinate care with families, referral sources, and external providers while ensuring timely documentation.
