Qualifications

• Bachelor’s degree in a human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing.
• Two years direct service experience serving persons with DD/IDD or related disabilities.
• Two years’ experience in healthcare, habilitative/rehabilitative or related settings.

Responsibilities

• Link participants with medical, social and education providers and services to address needs and goals.
• Conduct health, safety, social and environmental assessments.
• Participate in work groups, committees and meetings as requested.
• Demonstrate effective organizational and communication skills.
• Provide care management/coordination as part of the role.
