Job summary

CareStar, Inc. is seeking an Intensive Support Coordinator to assess needs, develop care plans, and connect clients with services in Clarke County, GA and surrounding areas. This role blends remote work with field visits, requiring flexibility and travel as needed.

The ISC Coordinator will join a mission-driven team focused on improving lives by coordinating healthcare, social supports, and services for individuals with complex needs. Bachelor’s degree and relevant experience are required.

Qualifications

• Bachelor’s degree in human service field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing.
• Two years’ direct service experience serving persons with DD/IDD or related disabilities.
• Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD) or similar.
• Experience in completing assessments, service plan development and monitoring.
• Excellent organizational and communication skills.

• Links participants with medical, social or educational providers and services to meet identified needs and goals.
• Conducts health, safety, social and environmental assessments per guidelines.
• Participates in work groups, community groups or committees and attends meetings as requested.
• Adheres to CareStar policies and procedures; follows the rules in job performance.
• Willingness and ability to travel; valid driver's license and car insurance as required by state law.
• Maintains confidentiality and follows Acceptable Use Policy for CareStar information systems.

Key Responsibilities

• Provides information and assistance to link participants with medical, social and/or
  educations providers and/or other programs and services that can provide needed
  services to address identified needs and achieve identified goals.
• Conducts health and safety, social and environmental assessments per established
  guidelines.
• Serves on work groups, community groups, committees, or other task forces and attends
  meetings as requested.
• Effective organizational and communication skills
• Experience providing care management/coordination preferred.
• Willingness and ability to work flexible schedule to meet needs of families including reliable transportation, valid driver's license and car insurance as required by state law, and the ability to travel.

• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
• Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Minimum Qualifications

• Bachelor’s degree in human service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field).
• Two year's direct service experience serving persons with DD/IDD or related disabilities.
• Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills
• Experience providing care management/coordination preferred.
