Job summary

CareStar, Inc. is seeking an Intensive Support Coordinator for Richmond County, GA, and surrounding areas. This full-time role combines remote work with field visits to provide care coordination and connect individuals with essential services.

The ISC Coordinator will assess needs, develop care plans, and collaborate with medical, social, and education providers. Travel is required to support families in the community.

Qualifications

• Bachelor’s degree in a human services field is required.
• Two years of direct service experience serving persons with DD/IDD or related disabilities.
• Two years of experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, or similar setting.
• Experience in completing assessments, service plan development and monitoring.
• Effective organizational and communication skills.
• Experience providing care management/coordination preferred.

• Provides information and assistance to link participants with medical, social and/or educational providers and/or other programs and services that can provide needed services to address identified needs and achieve identified goals.
• Conducts health and safety, social and environmental assessments per established guidelines.
• Serves on work groups, community groups, committees, or other task forces and attends meetings as requested.
• Adheres to the CareStar Rule in performance of job responsibilities.
• Understands and complies with CareStar Policies and Procedures.
• Maintains confidentiality as related to patient information.

• Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.

Minimum Qualifications

• Bachelor’s degree in human service Field such as Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing (or closely related field).

• Two year's direct service experience serving persons with DD/IDD or related disabilities.

• Two years’ experience in healthcare, habilitative/rehabilitative, residential, behavioral health setting (ICF/IDD), psychiatric inpatient hospital/facility, specialty clinic, special education department or similar setting.

• Experience in completing assessments, service plan development and monitoring.

• Effective organizational and communication skills

• Experience providing care management/coordination preferred.
