Qualifications

• High school diploma or equivalent and 3–4 years' intake experience.
• Bachelor's degree in a related field preferred.
• Delivers excellent customer service to caregivers and clinicians.
• Analyses data, prioritizes tasks, and ensures timely follow-through.
• Experience with practice management software (CentralReach preferred) and Excel.
• Exposure to continuous improvement work.

Responsibilities

• Coordinate scheduling of consultations, tours, and assessments.
• Serve as first-line support for routine scheduling questions.
• Enter and maintain intake records in the EMR with accuracy.
• Track case progress across intake stages.
• Assist with referral coordination and care navigation.
• Develop and refine workflows to improve efficiency.
• Ensure HIPAA compliance in all communications.
