Job summary

Gramian Consultancy is seeking an experienced Inpatient Medical Coding Specialist with an active CCS or CIC credential to contribute to AI-focused projects. You will review inpatient records, assign ICD-10-CM/PCS codes, and verify MS-DRG outcomes while applying UHDDS and official guidelines.

The role is fully remote in the United States, with emphasis on accuracy and data integrity. The ideal candidate has hands-on inpatient coding experience, strong knowledge of ICD-10-CM/PCS, MS-DRG

Qualifications

• Requires hands-on inpatient coding experience with active CCS or CIC credentials.
• Proficient in ICD-10-CM/PCS coding and MS-DRG methodology.
• Knowledgeable in UHDDS definitions, POA indicators, CC/MCC classification.
• Ability to review clinical documentation and resolve coding challenges.
• Strong written and verbal communication skills; high attention to detail.

Responsibilities

• Annotate and review inpatient medical records, including notes, operative reports, labs and imaging.
• Assign ICD-10-CM and ICD-10-PCS codes per guidelines.
• Identify principal and secondary diagnoses and sequence codes accordingly.
• Assign POA indicators and validate MS-DRG assignments (CCs/MCCs).
• Apply UHDDS definitions and official coding guidelines and AHA guidance.
• Identify HACs and documentation gaps; flag issues for resolution.
• Provide structured feedback to improve coding guidelines and AI model training.
• Ensure annotations align with current coding standards.
