Job summary

UnitedHealth Group is seeking an Inpatient Care Management Nurse to ensure appropriate utilization of health services and coordinate plans of care. This role offers remote work and requires strong clinical nursing experience and an RN license.

You will collaborate with Clinical Services and Medical Directors, perform reviews, and support discharge planning while adhering to SOPs and regulatory requirements.

Qualifications

• Current, unrestricted RN license in state of Residence.
• 3+ years of clinical nursing experience in acute inpatient, SNF/air/LTAC, ED, urgent care or similar.
• Experience in acute, long term care, or skilled nursing facilities.
• Proficiency in Windows, Excel, Outlook and clinical platforms.
• Designated workspace with secure high-speed internet at home.

Responsibilities

• Perform initial and concurrent review of inpatient cases using evidence-based criteria.
• Discuss cases with facility healthcare professionals to obtain plans-of-care.
• Collaborate with Medical Directors on utilization management.
• Support care progression discussions with Clinical Services team.
• Consult with Medical Director for complex cases and referrals.
• Apply clinical expertise in case discussions with case managers and physicians.
• Identify delays in care and escalate with MDs.
• Follow SOPs in end-to-end case management.
• Obtain clinical information to assess alternate levels of care.
• Facilitate timely and appropriate discharge planning.
