Alignment Health is hiring a Field Remote NP/PA to support the Care Anywhere program. The role involves in-home assessments, medication reviews, health screenings, and coordinating care with PCPs and specialists to prevent hospitalizations and disease progression.

With a Master’s in NP/PA and California licensure, you will educate patients, develop care plans with the medical team, and lead a multidisciplinary group in a fast-paced home-care setting.

Qualifications

• Master’s degree from an accredited NP/PA program.
• Active California NP or PA license is required.

Responsibilities

• 1) Conduct in-home assessments on Alignment members, medication review, health screenings; 2) Provide patient education by matching care desires with best care; 3) Identify diagnoses to be assessed in care management; 4) Partner with Regional Medical Officer and team to develop care plans; 5) Coordinate care with PCPs, specialists, and ancillary providers; 6) Lead broader clinical team including nurses, health coaches, social workers, and coordinators.
