Responsibilities

• Conduct in-home assessments for Alignment Health members, perform medication reviews and health screenings.
• Provide patient education by matching care desires with the best care.
• Identify diagnoses to be assessed in care management and active medical management.
• Collaborate with Regional Medical Officer and team to develop optimal care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead broader clinical team including nurses, health coaches, social workers, and care coordinators.
