Job summary

Evergreen Nephrology is seeking a program leader to own the day-to-day execution of practice onboarding and sustainment of partner relationships. You will manage the ECC onboarding sequence end-to-end and scale the program while maintaining timelines and accountability.

You will coordinate parallel implementation and clinical workstreams, facilitate sessions to decisions, and partner with internal teams to unblock practice-facing work and fix repeated process breaks.

Qualifications

• Experience owning onboarding program delivery across multiple practices.
• Strong meeting facilitation and cross-functional coordination.
• Ability to track deadlines and drive items to closure.

Responsibilities

• Coordinate parallel implementation and clinical workstreams, facilitate meetings, and drive each session to decisions.
• Maintain a single source of truth for deadlines and commitments, and drive open items to closure.
• Partner with internal teams to unblock practice-facing work and propose durable fixes for repeated process breaks.
