Job summary

Trillium Health Resources is hiring a Care Manager for the 1915i waiver to coordinate complex care for individuals with MH/DD/SA needs and other health requirements. The role emphasizes proactive assessment, multi-disciplinary collaboration, and adherence to waiver requirements.

Remote within NC, within 75 miles of the catchment, with standard hours and strong benefits including PTO and health coverage. NC licensure and LTSS experience strongly preferred.

Qualifications

• Fully licensed by NC governing board in Human Services professions; OR
• Fully licensed RN licensed in NC with MH/DD/SA experience.
• Master’s degree in a human service field with 1 year MH/DD/SA experience or substance abuse counseling supervision.
• Bachelor’s degree in human services with 2 years MH/DD/SA experience or supervised experience in counseling.
• Bachelor’s degree in non-human-services with 4 years MH/DD/SA experience or supervised experience in counseling.
• If LTSS duties apply, at least 2 years LTSS/HCBS coordination and care management experience.
• Must have a valid driver’s license.
• Must reside within 75 miles of Trillium’s catchment in NC and be able to travel within catchment.

Responsibilities

• Utilize person-centered planning methods to understand member needs.
• Provide education and support on rights and responsibilities.
• Complete discovery and information gathering as part of planning.
• Ensure assessments and plans are updated with life changes.
