Job summary

Trillium Health Resources is seeking a Care Transitions Care Manager to coordinate care for individuals transitioning between care settings and maintaining stability over time. This full-time, remote position serves the Mid State or Southern region within North Carolina.

The role requires NC licensure or RN licensure with MH/DD/SA experience, plus either a Master's in a human service field with 1 year MH/DD/SA experience or equivalent experience.

Qualifications

• Fully licensed by the NC governing board regulating Human Services professions, or an RN licensed in NC with MH/DD/SA experience.
• Master's degree in a human service field with one year of MH/DD/SA experience, or a substance abuse professional with one year of supervised experience.
• Bachelor's degree in a human service field with two years MH/DD/SA experience, or a substance abuse professional with two years supervised experience.
• Bachelor's degree in a field other than human services with four years MH/DD/SA experience, or a substance abuse professional with four years supervised experience.
• Two years LTSS/HCBS coordination experience; immunizations; valid NC driver's license; reside in specified counties within Mid State or Southern region.

Responsibilities

• Manage ongoing care for assigned individuals to support continuity and stability across services and settings.
• Coordinate services across providers and community resources to support long-term care management in a facility setting.
• Provide education on services and resources within the community.
• Link individuals to community resources and services for community-based integration.
• Engage individuals and families to support sustained participation in services and care plans.
