Qualifications

• Master's or Doctor of Nursing Practice degree required.
• Current NP license and unrestricted licensure in the applicable state.
• Board certification in Family NP or AGNP or related specialty is required.
• Experience with value-based ambulatory care and CMS guidelines is preferred.

Responsibilities

• Provide total care for assigned patients in collaboration with the care team.
• Assess patients, develop care plans, and manage acute and chronic conditions.
• Document diagnoses and care plans in the electronic medical record timely.
• Educate patients and families; coordinate with home health and community resources.
