IntellaTriage is seeking a compassionate registered nurse (RN) to provide after-hours hospice triage by phone from a remote setting. You will assess patient conditions, guide next steps, and coordinate with on-call teams to ensure timely care.

This role supports hospice and home health providers with a patient-centered approach, remote work setup, paid training, and potential to increase hourly rate within 6 months; PTO and benefits available.

Qualifications

• Active multistate RN license
• Hospice, palliative, or end-of-life care experience preferred
• Comfort with technology and EMR documentation
• Typing notes quickly in a fast-paced environment
• Strong communication and critical thinking skills

Responsabilités

• Provide telephone triage for hospice patients and families
• Assess patient conditions and determine appropriate next steps
• Coordinate with on-call teams to arrange care and resources
• Document all communications and interventions accurately
• Maintain a calm, professional demeanor during urgent calls
