IntellaTriage is seeking a compassionate registered nurse (RN) to provide after-hours hospice triage and care coordination from a remote setting. You will triage patients and families by phone, assess conditions, and coordinate with on-call teams to ensure timely interventions.

The role supports a remote, nationwide team with online documentation, EMR use, and a focus on compassionate patient-centric care. Training and benefits are included for full-time staff.

Qualifications

• Active multistate Registered Nurse license required.
• Hospice, palliative, or end-of-life care experience preferred.
• Comfort with technology and EMR for documentation.
• Able to type notes quickly in a fast-paced environment.
• Strong communication and critical thinking skills.

Responsibilities

• Provide telephone triage for hospice patients and families.
• Assess patient conditions and determine next steps.
• Collaborate with on-call teams to coordinate care and resources.
• Accurately document all communications and interventions.
• Maintain a calm, professional demeanor during urgent calls.
