Qualifications

• Active RN license (TX/Compact) required.
• CPR certification required.
• Prior geriatric home health care experience preferred.

• Valid driver's license required.

Responsibilities

• Deliver skilled nursing care at patient's home per physician orders.
• Collaborate with interdisciplinary team to develop and update care plans.
• Manage nursing treatments and clinical activities to support outcomes.
• Monitor patient progress and communicate findings to care team and family.
• Perform additional duties as needed.
