Qualifications

• Active RN license (TX/Compact).
• CPR certification required.
• Experience in geriatric home health care is a plus.

Responsibilities

• Deliver skilled nursing care in patients' homes per physician orders and plan of care.
• Collaborate with the interdisciplinary team to develop and update plans of care.
• Administer nursing treatments and manage clinical activities.
• Monitor progress and communicate findings to the care team, patient, family, and case manager.
• Additional duties as needed.
