Job summary

Pathfinder Home Health is seeking compassionate RN clinicians to join our Spring/Woodland/Magnolia, TX service area. You’ll use Roger app for HIPAA-compliant documentation, reducing charting time and boosting patient care focus.

As a full-time home health nurse, you’ll deliver high-quality skilled nursing care in patients’ homes, collaborate on care plans with the team, and leverage modern tools to improve outcomes while maintaining independence and work-life balance.

Qualifications

• Active RN license TX/Compact required.
• Active CPR certificate.
• Home health experience, geriatric focus.
• Valid driver’s license.

Responsibilities

• Deliver Skilled Nursing Care in the patient’s home as ordered.
• Collaborate on Plan of Care with the interdisciplinary team.
• Administer nursing treatments and manage clinical activities.
• Monitor patient progress and communicate with the care team.
