Home Care Providers of Texas Inc. in the SW Houston area seeks compassionate PRN RNs for in-home patient care, leveraging the Roger app to reduce charting time and boost independence.

You’ll join a team that values flexibility and local impact with Honor, Kindness and Patience. As a licensed home health nurse, you’ll provide skilled nursing in patients’ homes, collaborate on care plans with the interdisciplinary team, and use WellSky/Kinnser for electronic charting while meeting OASIS

Qualifications

• Active RN license (TX/Compact).
• Active CPR certificate.
• Home health experience, geriatric care preferred.
• Self-starter with strong organizational skills.
• Compassionate care approach.
• Valid driver’s license.

Responsabilités

• Deliver Skilled Nursing Care in the patient’s home per orders and care plan.
• Collaborate on Plan of Care with the interdisciplinary team.
• Manage treatments and interventions to support outcomes.
• Monitor progress and communicate findings with care team.
• Additional duties as needed.
