Job summary

Qualifications

• Current Montana Licensure Registered Nurse (RN).
• Current American Heart Association (AHA) Basic Life Support (BLS).
• Two (2) Years of nursing experience.

Responsibilities

• Works with physicians and multidisciplinary team members to develop a care plan for assigned patients.
• Monitors progress and identifies barriers hindering effective patient care.
• Engages in discharge planning process to ensure smooth transitions.
• Completes comprehensive assessments within regulated timelines and determines eligibility for case management.
• Develops and implements a case management plan with the member and care network.
• Performs ongoing monitoring of the care plan and documents interventions and goals.
