Responsibilities

• Retrieve and abstract medical records from EMR/EHR systems for HEDIS reporting.
• Document retrieval progress and findings for each EMR accessed.
• Communicate professionally with provider offices and coordinate follow-ups.
• Review medical records to extract specified HEDIS data elements (e.g. screening dates, labs).
