Job summary

Mochi Medical is expanding its virtual care model and hiring Health Coaches to support patients on GLP-1 medications. You’ll work with a multidisciplinary team to provide high-touch coaching for metabolism, nutrition, activity, and emotional health in a fully remote setting.

As part of a fast-growing startup, you’ll document sessions in our EMR, maintain privacy, and collaborate with physicians, NPs, and dietitians to drive meaningful patient outcomes.

Qualifications

• Active certifications: NBC-HWC, ACE-CHC, or CHES.
• At least 1 year of experience in health coaching, telehealth, or related roles.
• Proficient in EMR systems, scheduling tools, and video visits coordination.
• Understand scope of practice for health coaches and remote, startup environments.

Responsibilities

• Conduct 30-minute virtual coaching sessions to support lifestyle changes.
• Use motivational interviewing to set SMART goals and address setbacks.
• Carry a caseload and serve as accountability partner for patients.
• Educate on nutrition, exercise, sleep, stress, and behavior change without medical nutrition therapy.
• Document sessions in EMR using SOAP format and update the care team.
