Qualifications

• Board Certified Health and Wellness Coach required.
• 2+ years experience coaching or wellness education.
• Experience with digital care delivery or telehealth.
• Excellent written and verbal communication across channels.
• Proficient with Microsoft Office and online tools.

Responsibilities

• Use behavior change solutions consistently.
• Provide video consultations and messaging with patients.
• Design and support patient care plans using motivational interviewing and SMART goals.
• Manage a sizable patient caseload.
• Collaborate with cross-functional teams including physicians and therapists.
