Job summary

Sharecare is seeking a Health Coach to conduct telephonic coaching, enroll participants, and support a multi-disciplinary care team. You will engage members, provide education, and motivate lifestyle changes through structured workflows and documentation standards.

The role requires a Bachelor's in a health-related field, 1–2 years of relevant experience, proficiency in Microsoft Office, and the ability to learn and adapt to new topics such as nutrition and exercise.

Qualifications

• Bachelor's degree in health-related field required; NBHWC certification might substitute for the health-related part of the degree.
• 1-2 years of related experience; customer service experience preferred.
• Experience with benchmarks and contact center metrics; adherence to schedule.
• Proficiency in computer data entry and Microsoft Office.

Responsibilities

• Engagement (inbound and outbound) with eligible participants to inform and enroll in the program.
• Coaching (inbound and outbound) providing individualized behavior change coaching.
• Chase (outbound) contacting participants without a scheduled call.
• Coordinate delivery of services with nurses, pharmacists and vendors.
• Support operational aspects to meet customer requirements; comply with schedule.
• Maintain caseload and participate in daily/monthly meetings.
• Note and track progress and program efficacy.
• Adapt to schedule changes and multitask with participants and staff.
