Job summary

Sharecare is seeking a Health Coach to join a structured, high-engagement outreach model using an automated dialer system. You will connect with participants, provide education and motivation to progress toward health goals, and follow established workflows and documentation standards.

The role involves coaching on lifestyle topics, collaborating with a multidisciplinary team, and supporting program participants to achieve better health outcomes.

Qualifications

• Bachelor's degree in a health-related field required; NBHWC certification may substitute for the degree part in some cases.
• 1-2 years of related experience; customer service experience preferred.
• Experience with benchmarks and contact center metrics; adherence to schedule.
• Proficiency in computer data entry and Microsoft Office.
• Ability to coordinate telephonic communication while navigating software and entering data.
• Knowledge of lifestyle topics (smoking cessation, weight management, nutrition, exercise, stress reduction, sleep).
• Ability to work independently with minimal supervision and adapt to schedule changes.

Responsibilities

• Engagement with participants via inbound and outbound calls to enroll in the program.
• Provide individualized behavior change coaching to improve health outcomes.
• Coordinate with a multidisciplinary care team including nurses and pharmacists.
• Support scheduling, data entry, and documentation within the care platform.
• Attend daily and monthly meetings to present findings and insights.
• Track member progress and program efficacy.
