Qualifications

Responsibilities

• Manage a caseload of clients delivering education, advocacy, care coordination, and ongoing support.
• Conduct clinical intake via email or phone to assess needs and determine next steps.
• Collaborate with clinical and operations teams to provide appropriate provider recommendations and coordinate appointments.
• Support triage teams by handling inbounds and providing input on complex cases.
• Communicate clearly and compassionately with clients, caregivers, providers, and internal partners.
